+**active patch implied the commit files context was set up** (the only way to start a patch was to

+commits panel *without* touching `CommitFilesContext` (the deliberate decoupling), so its ref stays
+**nil**. Then any `Refresh({COMMIT_FILES})` while such a patch is active — e.g. ctrl+p → **Reset patch**
