This adds a feature to add additional authors to the previous commit,
this feature is supported by GitHub and GitLab and based on the format
should be self-evident to anyone reading the commit message.

Documentation rebuild: index.txt updates 
Removes most index.txt whitespace because whitespace is insignificant in
ronn index.txt files and POSIX shell makes it hard to use the right amount.

Re-orders git-clear/git-clear-soft and git-lock/git-locked due to
differences between the sorting by make and shell.

Although Github reserves the 'visionmedia' user space(a visit to there
will be redirected to 'tj'), it's better to
rename the links from visionmedia/git-extras to tj/git-extras

With hte manuals described in index.txt ronn will create hyperlinks of the (1) referenced words.
See http://rtomayko.github.com/ronn/ronn.1#LINK-INDEXES.
git-extras.html will now be browsable, actually anywhere else you use the referenced names ie. git-extras(1) as defined in index.txt will generate as hyperlinks in html while keeping the text versions rendereder similarly than they used to using exactly the same procedures as already documented.
