Leila Muhtasib 3d0b4feb80 graft: Require destination branch. Fix #23 
This is to avoid the old behaviour of automatically
merging into master.

Although Github reserves the 'visionmedia' user space(a visit to there
will be redirected to 'tj'), it's better to
rename the links from visionmedia/git-extras to tj/git-extras
